I Want to File a Consumer Complaint Illinois Department of Financial and Professional Regulation Division of Insurance

If you would like the Division of Insurance to investigate your complaint against an insurance company, producer or insurance agency, you may complete a consumer complaint form either electronically or in hard copy format. When we receive your completed form, we will initiate an investigation, assign a complaint file number and notify you. Please use this number on all future correspondence with this Division. If you are a health care provider, please use the Health Care Provider Complaint Form.
