Article XLII, Section 5/1206 of the Illinois Insurance Code, otherwise known as the Illinois Insurance Cost Containment Act, requires all Property-Casualty insurers licensed in Illinois under Class 2 and/or Class 3 to pay a reasonable fee established by the Director of Insurance sufficient to cover the total cost of the Department associated with the administration of this Article. The Article allows the Director to establish a schedule of fees for this purpose.

Illinois Administrative Code, Title 50, Chapter I, subchapter vv, Part 4201, Section 4201.20 Annual Fee sets forth the Schedule of Fees by which insurers are to determine their fee amounts owed. Section 4201.ILLUSTRATION shows the format of the Worksheet, which is to be completed and returned with the fee payment.

Please complete the enclosed Worksheet according to the schedule of fees and return this Worksheet with your company's payment on or before May 15, 1998. Return only the original Worksheet with your payment amount. Do not submit any copies of the original Worksheet as it has an embossed seal in the lower right corner of the page. Payments cannot be processed without the original worksheet.
