SPRINGFIELD, IL. -- Illinois Director of Insurance J. Anthony Clark has revoked the insurance producer license of Thomas V. Conwell of Lake Bluff effective June 13, 2003. Mr. Conwell had been licensed since March 8, 1993.
Mr. Conwell was licensed to sell life, accident and health and variable insurance. The license of Mr. Conwell was revoked by Director's Order from Hearing #3991 regarding the Department's Order of Revocation dated April 30, 2002. The stated Order of Revocation was issued as a result of an investigation which revealed that the licensee submitted documents containing non-genuine signatures of insureds to an insurer, rebated premiums, failed to repay a loan he obtained from an insured, and that his registration as a securities salesperson had been revoked by the Secretary of State Securities Division.
Mr. Conwell appealed the case for review under the Illinois Administrative Review Law; however, that case has been dismissed. A copy of the Director's Order is attached.
