Insurance License Revoked

SPRINGFIELD, IL. -- Illinois Director of Insurance J. Anthony Clark has revoked the insurance producer license of Antonio F. Sandoval of Chicago, Illinois, effective January 17, 2004. Mr. Sandoval had been licensed at least since February 20, 2002.

Mr. Sandoval was licensed to sell life insurance. His license was revoked as a result of the Licensee's failure to abide by a Stipulation and Consent Order he entered into with the Department in that he failed to pay the civil forfeiture of $500.00. The Stipulation and Consent Order was the result of an investigation that revealed a violation of the replacement regulation by Mr. Sandoval.

Under the Illinois Administrative Review Law he may have the right to appeal and request a stay of the Order. A copy of the Director's Order is attached.
