SPRINGFIELD, IL. -- Illinois Department of Insurance has denied the insurance producer license application of Frank O. Tartaglia of Rockford, Illinois effective February 15, 2004.
Mr. Tartaglia was applying for a license to sell property and casualty insurance. His license application was denied as a result of an investigation, which revealed that the Applicant was previously revoked in Illinois and failed to reveal the action on his application for a license; he failed to pay unpaid balances due an agency in the amount of $8,844.00 and two (2) insurers in the amount of $37,899.54 and $74,386.93 respectively; and he filed bankruptcy discharging fiduciary monies.
Under the Illinois Administrative Review Law he may have the right to appeal and request a stay of the Order. A copy of the Director's Order is attached.
