Insurance License Application Denied

SPRINGFIELD, IL. -- Illinois Acting Director of Insurance Deirdre K. Manna has denied the insurance producer license application of Alfred Imad Shuaibe of Phoenix, Arizona effective May 19, 2004.

Mr. Shuaibe was applying for a license to sell casualty and fire insurance. His license application was denied as a result of an investigation, which revealed that the Applicant failed to reveal on his application that he was previously denied a license twice in Wisconsin and that he has a child support obligation in arrearage. Additionally, he failed to respond to any Department communication regarding the above referenced matters.

Under the Illinois Administrative Review Law he may have the right to appeal and request a stay of the Order. A copy of the Director's Order is attached.
