Insurance License Revoked

SPRINGFIELD, IL. -- Illinois Acting Director of Insurance Deirdre K. Manna has revoked the insurance producer license of Henry Fleming, Sr. of Sauk Village, Illinois effective November 30, 2004. Mr. Fleming had been licensed since April 25, 1995.

Mr. Fleming was licensed to sell life, accident, and health insurance. The license of Mr. Fleming was revoked by Director's Order from Hearing #04-HR-102. The hearing was held regarding the Department's Order of Revocation dated January 15, 2004. The Order of Revocation was issued as a result of an investigation revealing Mr. Fleming had failed to report a 1993 felony within thirty (30) days and failed to reveal said felony on three licensing applications submitted to the Division over a nine year period.

Under the Illinois Administrative Review Law he may have the right to appeal and request a stay of the Order. A copy of the Director's Order is attached.
