Insurance License Revoked

SPRINGFIELD, IL. -- Illinois Acting Director of Insurance Deirdre K. Manna has revoked the insurance producer license of Lynette J. Perkins of LaSalle, Illinois effective January 7, 2005. Ms. Perkins had been licensed since March 18, 2003.

Ms. Perkins was licensed to sell accident, and health insurance. The licenses were revoked as a result of an investigation, which revealed that the Licensee submitted two (2) fraudulent applications to an insurer using a forged check; submitted an application on a non-existent consumer; and failed to facilitate and aid the Director in the examination.

Under the Illinois Administrative Review Law she may have the right to appeal and request a stay of the Order. A copy of the Director's Order is attached.
