Insurance License Revoked

SPRINGFIELD, IL. -- Illinois Acting Director of Insurance Deirdre K. Manna has revoked the insurance producer license of Robert L. Phillips of Calumet City, Illinois effective February 19, 2005. Mr. Phillips had been licensed since September 12, 2001.

Mr. Phillips was licensed to sell life, accident, health, fire, and casualty insurance. The licenses were revoked as a result of an investigation, which revealed that the Licensee obtained $20,336.50 by submitting fraudulent loan and cash surrender forms containing forged signatures of the consumers.

Under the Illinois Administrative Review Law he may have the right to appeal and request a stay of the Order. A copy of the Director's Order is attached.
