SPRINGFIELD, IL. -- Illinois Division of Insurance has denied the limited lines insurance producer license application of Berkley LaVelle of Streator, Illinois effective June 18, 2005.

His license application was denied as a result of an investigation, which revealed that Public Aid certified to the Division that the Applicant is more than 30 days delinquent in complying with a child support order in the amount of $14,170.75.

Under the Illinois Administrative Review Law he may have the right to appeal and request a stay of the Order. A copy of the Director’s Order is attached.
