Medicare Preferred Provider Organization (PPO)
Private Fee-For-Service (PFFS)
Medicare Health Maintenance Organization (HMO) is a type of Medicare Advantage Plan where Medicare contracts with an insurance company that contracts with a network of providers to provide care to people on Medicare. You pay a set copayment to the provider and must use the network of providers to receive care except in emergency or urgent situations.

The PPO provides coverage outside the organization's network of providers, subject to higher copayments and/or coinsurance.

Medicare Private Fee-For-Service (PFFS) has different rules than other Medicare Advantage Plans. For instance, there is no network of providers. You can go to any doctor or hospital that accepts the PFFS Plan's terms and conditions. The PFFS plan pays a set fee to the providers for your services and you pay a set fee to the provider.
