Hurricane Katrina Emergency Response Plan

Emergency Guidelines for Displaced Professionals from Alabama, Louisiana, and Mississippi

For licensed professionals from the states of Alabama , Louisiana and Mississippi who are temporarily displaced and interested in obtaining a professional license in order to practice in Illinois, the following provisions will apply:

1) All applicants must complete and submit the appropriate licensure application and supporting documents in full;

2) In bold letters label the bottom portion of the first page of the application

“Hurricane Katrina Displaced” ;

3) Application instructions require submission of the appropriate licensure application fee. However, if you are unable to pay the fee at this time, submit the application without the fee and the Division will grant an extension of the fee requirement for a period of 90 days;

4) Include correspondence indicating:

a. Personal circumstances due to Hurricane Katrina;

b. Former address;

c. Current temporary or permanent address, telephone

number and or e-mail (if applicable);

d. Request for an extension of up to 90 days for submission of the required application fee (if applicable);

e. Name and address of any professional organizations which the Division may contact to verify your license;

5) Provide copies of any documents you may have in your possession

demonstrating evidence of your professional license;

6) Submit the above documents to:

All applicable laws and rules for the profession for which licensure is sought must be met. However, if you are unable to obtain specific documentation due to the hurricane the Division of Professional Regulation will make every effort to assist you in the processing of your application.

The Division has established a “hotline” to respond to inquiries from displaced licensees. Please call 1-217-558-6188 and we will be happy to provide you further guidance and direction in this process. HOTLINE CALLS WILL ONLY BE ACCEPTED FROM PROFESSIONALS DISPLACED BY HURRICANE KATRINA.
