Disciplinary Actions Taken by the Division of Insurance |
SPRINGFIELD, IL - The Illinois Department of Financial and Professional Regulation (IDFPR) announced today that the Director of the Division of Insurance, Michael T. McRaith, signed the following disciplinary actions during the months of October and November 2006. The Division’s orders for each action are available by clicking on each name. Maxwell & Associates Insurance Agency & Joseph Maxwell, Chicago – insurance producer license revoked effective October 25, 2006. Maxwell & Associates Insurance Agency and Mr. Maxwell had been licensed since October 3, 2001 and March 22, 1991 respectively. Maxwell & Associates Insurance Agency and Mr. Maxwell were licensed to sell life, accident, health, fire and casualty insurance. The licenses were revoked as a result of an investigation, which revealed that the Business Entity and License failed to abide by and violated a Stipulation and Consent Order by failing to pay the civil forfeiture of $10,000.00 within 30 days as required by the Order. Emil Klanyac, Brookfield – insurance producer license application denied effective November 3, 2006. Mr. Klanyac was applying for a license to sell fire, casualty, accident, health, and life insurance. His license application was denied as a result of an investigation, which revealed that the Applicant was convicted of felony Possession of a Controlled Substance in 2003 and violated his probation which resulted in him serving a sixty day jail sentence. Cecilia Carlos, Plainfield – insurance producer license application denied effective November 6, 2006. Ms. Carlos was applying for a license to sell life insurance. Her license application was denied as a result of an investigation, which revealed that the Applicant was convicted of felony Retail Theft/Display Merchandise in 2005. Lisa A. White, Freeburg – insurance producer license revoked effective November 6, 2006. Ms. White had been licensed since April 10, 1986. Ms. White was licensed to sell life, accident, health, fire, casualty and variable insurance. The licenses were revoked as a result of an investigation, which revealed that the Licensee misappropriated $22,161.35 from an employer and failed to facilitate and the Director in the examination. Shawn P. Batiste, Waukegan – insurance producer application denied effective November 10, 2006. Mr. Batiste was applying for a license to sell fire and casualty insurance. His license application was denied as a result of an investigation, which revealed that the Applicant was convicted of a felony and failed to facilitate and aid the Director in the examination of his license application. Under the Illinois Administrative Review Law each may have the right to appeal the decision and request a stay of the Order. |
