Division of Insurance Disciplinary Actions |
Illinois Director of Insurance Michael T. McRaith has revoked the insurance producer licenses of the following insurance producers and companies in Illinois. The Division’s orders for each action are available by clicking on each name. Gregory D. Born of Wheaton, Illinois; Midland Insurance Group, Inc. of Naperville, Illinois; and Student Plans, Inc. of Naperville, Illinois effective November 19, 2005. Mr. Born, Midland Insurance Group, Inc., and Student Plans, Inc. had been licensed since 1979, 1991, and 1985 respectively. Mr. Born was licensed to sell life, accident, health, fire, and casualty insurance. Midland Insurance Group, Inc. was licensed as a Business Entity. Student Plans, Inc. was a licensed as a Business Entity and TPA. The licenses were revoked as a result of an investigation, which revealed that the Licensee and Business Entities entered into contracts of insurance as an insurer and transacted insurance business in this State and others without a proper certificate of authority. Jose R. Gonzales of Lockport, Illinois and La Mexicana Insurance Agency of Chicago, Illinois effective November 19, 2005. Mr. Gonzales and La Mexicana Insurance Agency had been continuously licensed since October 7, 2003 and February 13, 2003 respectively. Mr. Gonzales and La Mexicana Insurance Agency were licensed to sell fire and casualty insurance. The licenses were revoked as a result of an investigation, which revealed that the Licensee had improperly withheld premiums and multiple Premium Fund Trust Account record keeping violations. James Todd Gray of Macomb, Illinois effective November 21, 2005. Mr. Gray had been licensed since December 4, 2001. Mr. Gray was licensed to sell life, accident, health, fire, and casualty insurance. Mr. Gray’s license was revoked by Director’s Order from Hearing #04-HR-1021. The hearing was held regarding the Department’s Order of Revocation dated October 1, 2004. The Order of Revocation was issued as a result of an investigation revealing Mr. Gray had been convicted of felonies on two occasions (four (4) counts of Forgery and Unlawful Acquisition of a Controlled Substance in 2000 and Possession of a Controlled Substance in 2004), and failed to report the 2004 felony conviction within 30 days of the judgment. Ignacio Ramirez of Chicago, Illinois effective December 15, 2005. Mr. Ramirez had been licensed since June 18, 1999. Mr. Ramirez was licensed to sell life, accident, health, fire, and casualty insurance. Mr. Ramirez’s license was revoked as a result of an investigation, which revealed that the Licensee Improperly withheld $75,892.19 in premiums from an insurer and issued four (4) NSF checks to the insurer. Robert D. Mildenhall of Bluford effective December 8, 2005. Mr. Mildenhall had been licensed since November 23, 1999. Mr. Mildenhall is also known as Wade B. Palmer and Robert D. Mendenhall and has done business as Horse Creek Insurance Group. Mr. Mildenhall was licensed to sell life, accident and health insurance. Mr. Mildenhall’s license was revoked by the Department’s Order of Revocation dated November 8, 2005. The Order of Revocation was issued as a result of an investigation revealing Mr. Mildenhall was permanently barred from offering or selling securities in the state of Illinois and fined $30,000.00 by the Secretary of State Securities Division. The investigation further revealed that Mr. Mildenhall used his position as an insurance producer to convince Illinois consumers to invest monies in fraudulent investments and he failed to facilitate and aid in the investigation by failing to provide books and records to the Division of Insurance. Illinois Director of Insurance Michael T. McRaith has denied the licenses of the following applicants: Theodis Houston of Chicago, Illinois effective November 21, 2005. Mr. Houston was applying for a license to sell life insurance. Mr. Houston’s license was denied by Director’s Order from Hearing #05-HR-0581. The hearing was held regarding the Department’s Letter of Denial dated May 31, 2005. The Letter of Denial was issued as a result of an investigation revealing Mr. Houston had been convicted of felony Aggravated Kidnapping. Keith L. Turpin of Streamwood, Illinois effective November 21, 2005. Mr. Turpin was applying for a license to sell life, accident, and health insurance. Mr. Turpin’s license was denied by Director’s Order from Hearing #05-HR-0165. The hearing was held regarding the Department’s Letter of Denial dated January 20, 2005. The Letter of Denial was issued as a result of an investigation revealing Mr. Turpin had been convicted of felony Possession of a Controlled Substance in 2000 and failed to reveal the conviction on his original application for a license in 2002. Alejandro Pastrana of Bollingbrook, Illinois effective November 21, 2005. Mr. Pastrana was applying for a license to sell life, accident, health, fire, and casualty insurance. Mr. Pastrana’s license was denied by Director’s Order from Hearing #04-HR-0941. The hearing was held regarding the Department’s Letter of Denial dated September10, 2004. The Letter of Denial was issued as a result of an investigation revealing Mr. Pastrana had been convicted of felony Attempted Burglary in 2001. Roger L. Kern of Dixon, Illinois effective November 21, 2005. Mr. Kern was applying for a license to sell life, accident, and health insurance. Mr. Kern’s license was denied by Director’s Order from Hearing #05-HR-0204. The hearing was held regarding the Department’s Letter of Denial dated February 4, 2005. The Letter of Denial was issued as a result of an investigation revealing Mr. Kern had been convicted of two (2) counts of felony Aggravated Criminal Sexual Abuse in 2004. |
