Notice to Funeral Directors Clarifying Order of July 16 |
Furthermore, the Department would like to clarify the scope of the documents requested. The Department seeks documents reflecting the burial of a particular deceased person in a particular location at Burr Oak Cemetery and any documents reflecting the terms of burials at Burr Oak Cemetery, including copies of written statements prepared pursuant to Section 15-75(b)(28)(C) of the Act indicating that a deceased person was to be buried at Burr Oak Cemetery. Such statements are sometimes referred to as funeral director services agreements or contracts or statements of services. If you are a licensee that has no documents related to Burr Oak Cemetery, please provide written notice to the Department of this fact by e-mailing fpr.cemetery@illinois.gov.
