Dental Professions

 

The Department of Financial and Professional Regulation, Division of Professional Regulation, requires the assistance of Illinois licensed Dentists and Dental Hygienists in responding to the census (see links below) in order for the Department to obtain relevant information regarding the availability of dental services within the State.

Please note the following prior to taking the online survey:

  • Since the survey is on a secured Web site, either a pop-up security prompt or warning regarding the Web site certificate may appear. If so, please click the response to CONTINUE to the survey.
  • When navigating between fields on the forms, DO NOT use the enter key to move between fields.

    Please be advised that Controlled Substances Registration applications and printed licenses will no longer specifically reference or include the 'N' designation. Under the Illinois Controlled Substances Act narcotic and non-narcotic schedules are combined and the 'N' designation is not needed. The lack of 'N' designation does not prevent or affect the ability of Illinois licensed practitioners to prescribe, dispense or order narcotic drugs to the extent allowed by law. The DEA is aware of this change. Questions regarding 'N' designations on DEA registrations should be directed to the DEA at their website(www.deadiversion.usdoj.gov) or by phone at 312-353-7875.
