Welcome Consumer Installment Loan Act Licensees!

Please click Here to view the instructions for completing your Annual Report. Once the instructions appear, we recommended to you print a copy for reference.

Please click Here to print out the Blank form.  You can use this as the paper form for entry OR fill out the form with information from your calculations and then log into the application and enter the figures from the blank form onto the annual report application (see next paragraph).

Please click Here to invoke the annual report application. Once you finalize your annual report in this application, you will still be able to view it, print it, and print the affidavit but you will not be able to change it. If you need to make changes after finalizing your annual report you will need to contact the Consumer Credit Section at (312) 814-5145.

The final Annual Report must be submitted by March 1.

A FINE OF $25.00/DAY MAY BE ASSESSED FOR LATE REPORTS
