To become an Education Provider you must complete the Request for Certification of a Pre-Licensing Course or the Request for Certification of a Continuing Education Course depending on the type of course you will be giving. If you are a non-resident provider and your state is a member of the NAIC Midwest Zone you may apply for education certification by using the NAIC Midwest Zone Continuing Education Filing Form. Courses may be a maximum of 15 hours.
Filing forms for Self-Study classes must be accompanied by a timed outline. If the self-study course is a publishers course you must verify with the Division of Insurance that the course has been previously approved.
Fees must accompany all filing forms. New Course fees are $50.00, Provider Fees are $1,000.00. Checks, cashiers checks or money orders should be made payable to the Director of Insurance.
INSTRUCTOR REQUIREMENTS:
Instructors are required to hold a Bachelor's degree or have three years experience in the course subject matter. Providers must maintain evidence of such qualifications while the instructor is actively engaged in instructing the course and for one year thereafter.
REPORTING REQUIREMENTS:
Providers must report students who have successfully completed a pre-licensing or continuing education course to the Director on computer diskette or other electronic method of transfer prescribed by the Director and in the specifications established by the Director. Each list shall be received by the Director within ten days following the end of the week in which the course was completed.
