If one or more licenses are found you will see a table containing information
about professionals who match the criteria entered in the search boxes of
either the Name Search page or the License Search page. The results table
will include the licensee's name, license number, practicing city and state,
original license date, current expiration date of the license and for some
professions, some other information. For example Medical Temporaries, the
program name is shown. A license qualifier will only show when a search
is completed on a profession title which is licensed and has a special qualifier
such as Optometry, Dental Specialty and Advanced Practice Nurse. For example,
Advanced Practice Nurses may be qualified to practice under one of several
specialties; the specialty for a specific license is listed under qualifier.
The table will also have an indicator of whether or not the licensee has
ever been disciplined while practicing in that profession. If a professional
has never been disciplined, there will be an "N" in that box.
However, if a professional has a disciplinary action on file with the Department
a grey button with the letter "Y" will appear. Click on the button
to reveal a brief description of the actions which led to the discipline,
the name of the disciplinary action,and the start and end dates of the disciplinary
