License Look-up - A search engine located on the IDPR website
which enables its users to search the IDPR profession database. A search
query returns results which contain the following profession information:
name, DBA/AKA, license number, license status, city, state, original date,
current expiration, other special fields, whether or not the individual
has ever been disciplined and a description of the disciplined action.
Name Search - a feature within the license look-up search engine which
allows the user to look up, by name, a professional who is licensed within
the State of Illinois and regulated by IDPR
License Number Search - a feature within the license look-up search
engine which allows the user to look up, by license number, a professional
who is licensed within the State of Illinois and regulated by IDPR.
Original Date - the calendar date the professional received his/her
initial license to practice their profession in the State of Illinois.
Current Expiration Date - the calendar date the existing profession
license is due to expire.
Discipline Start Date - the calendar date the professional received
his/her discipline.
Discipline End Date - the calendar date the imposed discipline is due
to expire. License Qualifier - A license qualifier further
identifies a licensee's practicing specialty. For example, Advanced Practice
Nurses may be qualified to practice under one of several specialties; the
specialty for a specific license is listed under qualifier. The professions
displaying this information which have special qualifiers are Dental Specialists,
Advanced Practice Nurses and Optometrists. Monthly Disciplinary
Report - a monthly report published by IDPR detailing disciplinary action
taken by the Department. Each report is a listing of all licensees disciplined
by the Department within a given month. The information includes the name
of the disciplined profession, the city where they were practicing at the
time of action, the discipline imposed and a brief description of the reason
for the discipline.
JCAHO - Joint Commission of Accreditation of Healthcare Organizations.
An independent, not for profit organization that evaluates and accredits
health care organizations in the United States.
NCQA - National Committee for Quality Assurance. An independant, nonprofit
organization that evaluates and reports on the quality of the nation's managed
care organizations.
