the following orders:

fine after having been terminated for failing to rectify a dishonored
check submitted to the agency.

The basis for the agreement was due to the previous discipline of the
applicant's Illinois insurance license.

application by certifying that he complied with the completing the continuing

to pay a fine and late fee for each deficient credit hour.
