Division of Insurance Disciplinary Actions
February 2006

 

Illinois Director of Insurance Michael T. McRaith has signed orders disciplining three insurance producers in Illinois in February.  The Division’s orders for each action are available by clicking on each name.

Sham A. Ahuja, Westmont, was denied an insurance producer license to sell property and casualty insurance, effective January 18, 2006. Mr. Ahuja’s license application was denied as a result of an investigation, which revealed that the Applicant was convicted of felony Unlawful Restraint and felony Criminal Sexual Abuse in April of 2004.

Quick-N-Sure 2 LLC and Alejandra Viveros, Chicago, had the Business Entity license and the insurance producer license revoked, effective January 21, 2006. Quick-N-Sure 2 LLC and Mr. Viveros had been licensed since September 10, 2005 and November 20, 2003 respectively. Mr. Viveros was licensed to sell fire and casualty. The licenses were revoked as a result of an investigation, which revealed that the Business Entity and Licensee improperly withheld premiums due a Premium Finance Company (PFC), issued NSF checks to the PFC for the premiums, and failed to facilitate and aid the Director in the examination.

Thomas L. Stasiek, Chicago, had his insurance producer license revoked, effective January 23, 2006. Mr. Stasiek had been licensed since March 30, 2005. Mr. Stasiek was licensed to sell life, accident, health, fire, and casualty insurance. Mr. Stasiek’s license was revoked as a result of an investigation, which revealed a felony conviction Possession of a Controlled Substance in 2005 and failing to report said conviction on a 2005 reinstatement application.

Under the Illinois Administrative Review Law each licensee may have the right to appeal and request a stay of the Order.
