Disciplinary Actions Taken by the Division of Insurance

 

 

SPRINGFIELD, IL - The Illinois Department of Financial and Professional Regulation (IDFPR) announced today that the Director of the Division of Insurance, Michael T. McRaith, signed the following disciplinary actions during the months of August and September 2006. The Division’s orders for each action are available by clicking on each name.

Bryan K. Brown, Pekin – insurance producer license revoked effective August 31, 2006.  Mr. Brown had been licensed since September 27, 2004.  Mr. Brown was licensed to sell life, accident, and health.  The license was revoked as a result of an investigation, which revealed that the Licensee is more than thirty days delinquent in complying with a child support order and he failed to notify the Division of his change of address within thirty days of the change.  Additionally, the Licensee failed to facilitate and aid the Director in an examination.

James McRae Izett, Lilburn, GA insurance producer license revoked effective September 14, 2006. Mr. Izett had been licensed since February 8, 2005. Mr. Izett was licensed to sell life, accident, health, fire casualty, and variable insurance. The licenses were revoked as a result of an investigation, which revealed that the Licensee failed to reveal his bankruptcy on his 2005 non-resident application and then failed to timely provide documentation as requested.

Panga Keo, Chicago – insurance producer license denied effective September 14, 2006. Mr. Keo was applying for a license to sell life. His license application was denied as a result of an investigation that the Applicant failed to facilitate and aid the Director in an examination.

Jacqueline Narvaez, Crestwood – insurance produce license revoked effective September 18, 2006. Ms. Narvaez had been licensed since May 8, 2002. Ms. Narvaez was licensed to sell life, accident, health, fire and casualty insurance. The licenses were revoked as a result of an investigation, which revealed that the Licensee improperly withheld $3,086.15 in premiums, failed to notify the Division of a change of address with 30 days and failed to facilitate and aid the Director in the examination.

Under the Illinois Administrative Review Law each may have the right to appeal the decision and request a stay of the Order.
